Health Records Business Process

This is a simple health records business process created using ProcessMaker, a really simple and easy to use business process management solution. This process has two entry points. One entry point allows patients to create a records release request to transfer health records from one heath provider to another health provider. Another entry point allows users to fill the health background online and submit it to clinic.
